If all the documents needed to start the production of the PCBs are ready and complete, you should pay for: 

* PCB production equipment costs; 
* costs of the stencil necessary to draft the solder paste on the PCB;
* pick&place machinery setting costs;

This kind of costs are needed only with the first order and never with the followings, if there will be no modifies to the electronic board's design.
